The goal of this series is to provide insight and support for youth, families and professionals who are involved in this transition.       In this first episode, Sylvia Pelletier provides an overview of health care transition. Donna Dunlop and Misty Martinez Bohannon join us to share their experiences navigating this complex process as caregivers. Donna is a parent of a young adult with complex medical needs who has completed the transition to adult providers. Misty is a parent of a young adult in the midst of transition. During the conversation, Misty and Donna outline recommendations for transition planning and highlight what has been helpful for their families.
