    • Pelvic Floor Tension vs Weakness: Why This Model Is Failing Your Patients

      Dr. Becky challenges the common pelvic floor therapy model that sorts patients into “weak/lengthened” versus “tight/tense/shortened” buckets, arguing these are symptoms rather than meaningful diagnoses or root causes. She explains how this binary approach can keep patients stuck with partial improvements or lifelong symptom-management tools and notes that tightness and weakness can coexist. The episode critiques relying on Kegels—both in traditional strengthening plans and in the “kill the Kegels” movement—stating Kegels don’t train automatic, functional pelvic floor behavior during real-life tasks. Using examples like stress incontinence and dyspareunia, the host emphasizes asking why the pelvic floor is compensating and recommends “getting out of the pelvis” to assess broader drivers such as pressure management from the rib cage/abdomen, pelvis biomechanics, foot/ankle function and the kinetic chain, nervous system/limbic influences, gut issues, and habits, posture, loads, and training. Dr. Becky introduces the ROOT acronym: remember to find the Root cause; Observe the patient’s story, symptoms, goals, and functional movement; Optimize the identified system limitation to restore automatic pelvic floor function; and Transform by building capacity and resilience through functional movement and loading.
