Every episode covers what first-gen docs need to know:

  • Contract negotiation and career decisions
  • Paying off debt without family financial guidance
  • Building wealth from scratch
  • The questions you don't know to ask (but your colleagues already knew)

Real strategies from doctors who had to figure it out on their own.

  • 2012: The Year I Stopped Asking For Permission. #485
    27 Oct 2026

SEND US A TEXT MESSAGE!!! Let Drs. Nii & Renee know what you think about the show!

Most people can point to one year that changed everything.

For me that year was 2012.

I finished my trauma surgery fellowship in Miami. Had a contract negotiation go sideways when a trauma director yelled at me over the phone. Called Renee. She said one sentence. And then I went silent.

    That silence changed my life.

In this episode I'm taking you back to 2012 — the year I said yes to everything that scared me and stopped waiting for someone to tell me who I was supposed to be.

    You’ll hear:

→ The contract negotiation that got me yelled at — and why I was right to fight back

→ The one sentence Renee said that I haven't stopped thinking about since

→ Why I called a locums company and never looked back

→ Landing in Ghana for the first time as a surgeon — same airport, completely different man

→ Finding out my father had prostate cancer while I was still a fellow — and what I did about it

→ Why 2012 was the year I stopped being someone's trainee and started being myself

This episode is for every resident finishing training, every early attending figuring out their next move, and every first-gen professional who has ever waited for someone else's permission to begin.

The phone call with Renee didn't change my life. The silence after it did.

  • A Dental Student Just Ran Out of Money. The Government Did This. #484
    13 Oct 2026

SEND US A TEXT MESSAGE!!! Let Drs. Nii & Renee know what you think about the show!

A first-year dental student posted a video on Instagram that stopped me cold.

He's run out of federal funding. He's run out of private funding. He started a GoFundMe just to stay in school.

The culprit: The One Big Beautiful Bill Act.

And here's why this felt so personal. This didn't happen to me. But it could have. Renee and I both needed private loans AND co-signers to finish our education. Without that access, there would have been a gap.

    You’ll hear:

→ The dental student's story and why it's just the beginning

→ What the Big Beautiful Bill actually does to graduate and professional student loans

→ The real numbers: $200,000 lifetime federal cap for medical and dental students

→ Who gets hurt most? First-gen students from underserved communities

→ Why medical schools need to step up and the risk of predatory institutional lending

→ 3 tips for medical and dental students navigating this right now

    KEY FACTS:

    • Grad PLUS loans eliminated for new borrowers: July 1, 2026
    • Medical/dental/law students: $50,000/year | $200,000 lifetime federal cap
    • Graduate students: $20,500/year | $100,000 lifetime cap
    • Previous system: Students could borrow up to full cost of attendance

  • 20 Years Later — What I'd Tell Every Medical Student. #483
    29 Sep 2026

SEND US A TEXT MESSAGE!!! Let Drs. Nii & Renee know what you think about the show!

Twenty years after medical school, I went back to Kansas City with Renee and our boys. Walking through the place where it all began brought back memories of my father driving me there from New Jersey, the first time I was rejected from medical school, and the life I could never have planned...

In this episode, I’m sharing things I wish I could tell the student I was back then.

    You’ll hear:

→ Why getting rejected from medical school the first time affected my confidence even after I got in

→ What my father’s support on that drive to Kansas City means to me now that he has severe dementia

→ How Renee and I met, became friends, and eventually built a life together

→ Why I wish I had learned to speak up sooner instead of trying to please everyone

→ Why becoming a doctor gives you options, and why you should keep hold of the keys to your own career

If you’re a pre-medical student, a medical student, or the first doctor in your family, I hope this reminds you that one setback does not decide where your life goes.
